INTRODUCTION The cortical degenerative dementias of focal origin (CDDFO) form a heterogeneous group of processes which affect the brain initially in a circumscribed manner. They are commoner than was thought and may form 10% to 20% of the total number of degenerative dementias seen by specialist departments. DEVELOPMENT. The CDDFO are divided into two large groups. In the first group the lesions are rostral. These are the frontotemporal dementias which make up 74% of our observations of CDDFO. The clinical findings depend on the site of the lesion. The most frequent picture is of frontal dementia, which has very sensitive, specific criteria of diagnosis. The second commonest group is non-fluent progressive dysphasia. These two groups form 60% of our cases. Progressive primary anarthria and semantic dysphasia are much less frequent. The most important morphological finding, common to all these ‘anterior or rostral’ dementias, is the absence of neurofibril degeneration and senile plaques. In the second group the caudal region of the brain is involved. The lesions spread from the occipital region to the parietotemporal zones. This shows as complex changes in vision and other cognoscitive deficits. This type of disorder is known as progressive visual agnosia or posterior cortical atrophy (PCA). CONCLUSION. This paper deals with the analysis of this clinical condition, based on personal experience and the analysis of published cases. The aim is to try to clarify the nature of PCA and discuss its physiopathology
